The draft UIC permit includes construction, operation, monitoring, and closure requirements for the injection well . As required by Title 40 of the Code of Federal Regulations, Section 124 . 9, the administrative record, which includes the ap- plication, draft permit, and other information, is available for public inspection at the Region 9 EPA Library, 6th floor, 215 Fremont Street, San Francisco, CA 94105. The draft permit and fact sheet are available for review at the Lodi Public Library, 201 W. Locust Street, Lodi, CA and at the Stockton Public Library, 605 N. E1 Dorado, Stockton, CA. The following comment period and proposed public hearing will provide EPA with the information and public opinion neces- sary to issue, modify, or deny the permit. The public comment period closes on November 2 , 1989 . A public workshop will be held on October 25, 1989 from 7 : 00 p.m. to 10: 00 p.m. at the Lodi Motor Inn, 1140 South Cherokee Lane, Lodi, California. The purpose of the workshop is to allow any interested parties to review information and ask questions regarding the proposed issuance of the permit. The public hear- ing will be held on October 26, 1989 from 7: 00 p.m. to 10: 00 p.m. also at the Lodi Motor Inn. Requests to present oral testimony should be filed by Oc- tober 19, 1989. If significant public interest in holding the workshop and hearing is not expressed by October 19, 1989, EPA intends to forgo the hearing and workshop. Those who requested a hearing will be notified in writing. If the public workshop and hearing are cancelled, any written comments or objections to any or all of the draft permit conditions, pursuant to 40 CFR §124 . 11, may be submitted no later than November 2, 1989. Requests for a public hearing should include the following infor- mation: 1) the name, address, and telephone number of the individual, or- ganization, or other entity requesting a hearing; 2) a brief statement of the requesting person's interest in the proposed action and of information that the requesting person intends to submit at such hearing; and 3) the signature of the individual making the request, or if the request is made on behalf of an organization or other en- tity, the signature of a responsible official of the or- ganization or other entity.
