(b) Anticipated Noncompliance. The permittee shall give <br /> advance written notice to the Director of any planned <br /> changes in the permitted facility or activity which may <br /> result in noncompliance with permit requirements. <br /> (c) Twenty-four Hour Reporting. <br /> (1) The permittee shall report to the Director any non- <br /> compliance which may endanger health or the en- <br /> vironment. Any information shall be provided <br /> orally within 24 hours from the time the permittee <br /> becomes aware of the circumstances. The following <br /> shall be included as information which must be <br /> reported orally within 24 hours: <br /> (i) Any monitoring or other information which indi- <br /> cates that any contaminant may cause an endan- <br /> germent to an underground source of drinking <br /> water. <br /> (ii) Any noncompliance with a permit condition, or <br /> malfunction of the injection system, which may <br /> cause fluid migration into or between under- <br /> ground sources of drinking water. <br /> (2) A written submission shall also be provided within <br /> five days of the time the permittee becomes aware <br /> of the circumstances. The written submission shall <br /> contain a description of the noncompliance and its <br /> cause; the period of noncompliance, including exact <br /> dates and times, and if the noncompliance has not <br /> been corrected, the anticipated time it is expected <br /> to continue; and steps taken or planned to reduce, <br /> eliminate, and prevent recurrence of the noncom- <br /> pliance. <br /> (d) Reporting of Other Non-Compliance. The permittee shall <br /> report all other instances of noncompliance not other- <br /> wise reported at the time monitoring reports required <br /> in Part II, Section D of this permit are submitted. <br /> The reports shall contain the information listed in <br /> Permit Condition 12 (c) (2) above. <br /> (e) Other Information. When the permittee becomes aware <br /> that he failed to submit any relevant facts in the per- <br /> mit application or submitted incorrect information in a <br /> permit application or in any report to the Director, <br /> the permittee shall submit such facts or information in <br /> writing within 10 days. <br /> (f) Report on Permit Review. Within thirty (30) days of <br /> receipt of this permit, the permittee shall report to <br /> the Director that he has read and is personally <br /> familiar with all terms and conditions of this permit. <br /> 8 <br />
