The appropriate logs and tests as specified in Part III, Section A, of this permit, shall be <br /> conducted auring the drilling and construction of the well. A descriptive report interpret- <br /> ing the results of such logs and tests shall be prepared by a knowledgeable log analyst and <br /> submitted to the Director. <br /> S. Construction Changes <br /> The permittee shall give advance notice, to the Director for review, of any planned changes <br /> in the construction of the permitted injection well. Unless approval is given by the Direc- <br /> tor, no significant deviations from the above construction plan will be authorized. <br /> B. OPERATION <br /> During the period beginning on the effective date of this permit and ending ten years from <br /> the effective date of this permit, the permittee is authorized to inject only non-hazardous <br /> pretreated process wastewater generated by the onsite meat processing plant and extracted <br /> groundwater into the injection well under the following conditions: <br /> 1. Iniection Formation <br /> Injection shall be limited to the Domengine Formation or lower formations. In no case <br /> shall injection be permitted in the Nortonville formation of higher formations. <br /> 2. Confining, Laver <br /> The injection formation is confined by rock of the Nortonville Formation which im- <br /> mediately overlies the Domengine Formation. <br /> 3. Quality of Iniected Waters <br /> The chemical make-up of the injected waters will be limited to concentrations which assure <br /> that no violation of EPA Primary Drinking Water Standards as set forth in 40 CFR 144.12 <br /> occurs in the receiving waters. <br /> 4. Injection Pressure Limitation <br /> Injection pressure at the wellhead shall not exceed a maximum which shall be calculated so <br /> as to assure that the pressure in the injection zone during injection does not initiate new <br /> fractures or propagate existing fractures in the injection zone. In no case shall injection <br /> pressure initiate fractures in the confining zone or cause the movement of injection or for- <br /> mation fluids into an underground source of drinking water. The maximum injection pres- <br /> sure, measured at the wellhead, shall not exceed 810 psig. <br /> S. Injection Pressure Modification <br /> No increases in injection pressure will be permitted. <br /> 6. Additional In iection Limitation <br /> Injection between the outermost casing protecting underground sources of drinking water <br /> and the well bore is prohibited. <br /> 11 <br />
