The LLOC found tank annular breaches in the 87 <br /> and 91 turbine sumps . CGRS will provide all labor and materials to make the repairs per the following Scope <br /> of Work . CGRS , Inc . is a California Licensed Contractor, License #803616 --- A/C-61 / D40/D64/ HAZ . All <br /> technicians performing tests and certifications are individually certified by the equipment manufacturers and <br /> hold applicable International Code Council ( ICC ) certificates . <br /> CGRS will obtain a repair permit from the San Joaquin County Environmental Health Department <br /> ( CUPA) . <br /> Scope of Work : <br /> Task # 1 - Pre UST tank top repair activities: <br /> • CGRS will obtain permits from CUPA <br /> • Order parts <br /> Task #2 — UST 87 & 91 tank annular repair <br /> • Saw cut and demo 10 ' x 10' area around turbine sumps to accommodate new NOV piping <br /> penetrations for the product supply line , vapor return , & vent lines <br /> • CGRS will remove all risers , piping , and turbines in both sumps to gain access to the tank <br /> annular breaches <br /> • CGRS will make fiberglass repairs of the tank annular leaks in the 87 and 91 turbine sumps <br /> CGRS will install new penetration fittings on all piping and electrical fittings in both sumps <br /> • Call for inspections as necessary <br /> • Perform SB989 re-testing per CUPA requirements <br /> Task #3 — UST 87 & 91 turbine sump reassembly <br /> • CGRS will reinstall the existing 87 turbine assembly with a new 2HP turbine <br /> CGRS will install new flex lines and plumbing as necessary <br /> • CGRS will install a new 2HP Red Jacket Turbine assembly in the 91 turbine sump <br /> • CGRS will install new flex lines and plumbing as necessary <br /> • CGRS will purge product lines <br /> • Preform all required SB 989 testing <br /> • Call for inspections as necessary <br /> Task #4 — Concrete Replacement <br /> • CGRS will backfill excavation with existing pea gravel <br /> • CGRS will place #4 rebar on center each way <br /> • CGRS will place 250OPSI concrete over excavation <br />
