SAN J OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> GILLIES TRUCKING INC 3931 NEWTON RD, STOCKTON May 18, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS) annually by the due date, January 15. The business plan has not been <br /> submitted for 2019, 2020, and 2021. The business plan information was not reviewed and resubmitted in the <br /> California Environmental Reporting System (CERS) annually by the due date since 12/13/17. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 10 HSC 25508.1(a)-(f) Failed to electronically update the HMBP within 30 days of substantial change in operations. <br /> OBSERVATION: During the inspection, two 55 gallon drums of degreaser were found on site. The degreaser has <br /> been at this facility greater than 30 day (actual date of arrival was not available). The business has not updated and <br /> submitted the information within 30 days. <br /> REGULATION GUIDANCE: Within 30 days of one the following events, business shall electronically update the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> FA0009366 PR0519582 SCO01 05/18/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 6 of 7 Hazardous Materials Program OIR <br /> 1868 E.
