OS126 ( 2 ` Environmental Health Department SAN JOAQUIN Underground Storage Tank Program Inspection Report Facility Name : Facility Address : Date: AM PM HAMMER/15 FOOD #83113 1 3250 W HAMMER LN , STOCKTON May 26 , 2020 SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) Item # Remarks 208 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days . OBSERVATION : The designated operator failed to check and note all required items on the visual inspection reports . The incorrect and missing information included the following from the inspection reports : - The 10/15/2019 , 11 / 13/2019 and 4/ 1 /2020 DO reports did not include the date signed by the ownner/operator -The 01 /8/2020 , 2/5/2020 , 7/25/2019 , 8/20/2019 and 9/18/2019 DO reports did not include the owner/operator name . REGULATION GUIDANCE : During the visual inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION : Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports . This is a minor violation . 210 CCR 2715 (c) No employee present that were trained in the operation and maintenance of the UST system by a DO . OBSERVATION : The designated operator employee training for Sally Moreno was not performed and NOT CURRENT. REGULATION GUIDANCE : The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees . The training shall include , but is not limited to: 1 . Operation of the UST system in a manner consistent with the facility's best management practices 2 . Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3 . Employee's role with regard to spills and overfills as specified in the facility's response plan 4 . Name of the contact person (s) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies , leak detection , monitoring alarms, spills and overfills . At least one of the facility employees present during operating hours shall have current training . CORRECTIVE ACTION : Ensure that employees have been trained by the designated operator, maintain the list on site , and submit a copy of the training records to the EHD . This is a minor violation . Overall Inspection Comments : An inspection report was provided to the facility operator on the day of inspection . The report was amended and replaces the initial report . " Violation #204 ( now 205) has been removed .
