OS126 ( 2 ` Environmental Health Department <br /> SAN JOAQUIN <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name : Facility Address : Date: <br /> AM PM HAMMER/15 FOOD #83113 1 3250 W HAMMER LN , STOCKTON May 26 , 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR - Notice to Comply) <br /> Item # Remarks <br /> 208 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days . <br /> OBSERVATION : The designated operator failed to check and note all required items on the visual inspection <br /> reports . The incorrect and missing information included the following from the inspection reports : <br /> - The 10/15/2019 , 11 / 13/2019 and 4/ 1 /2020 DO reports did not include the date signed by the ownner/operator <br /> -The 01 /8/2020 , 2/5/2020 , 7/25/2019 , 8/20/2019 and 9/18/2019 DO reports did not include the owner/operator <br /> name . <br /> REGULATION GUIDANCE : During the visual inspection , the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION : Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports . <br /> This is a minor violation . <br /> 210 CCR 2715 (c) No employee present that were trained in the operation and maintenance of the UST system by a DO . <br /> OBSERVATION : The designated operator employee training for Sally Moreno was not performed and NOT <br /> CURRENT. <br /> REGULATION GUIDANCE : The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees . The training shall include , but is not limited to: <br /> 1 . Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2 . Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3 . Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4 . Name of the contact person (s) for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies , leak detection , monitoring alarms, spills and overfills . At least <br /> one of the facility employees present during operating hours shall have current training . <br /> CORRECTIVE ACTION : Ensure that employees have been trained by the designated operator, maintain the list on <br /> site , and submit a copy of the training records to the EHD . <br /> This is a minor violation . <br /> Overall Inspection Comments : <br /> An inspection report was provided to the facility operator on the day of inspection .
