I contacted Mike Alegre, Battalion GAITAN <br /> Chief for Lodi Fire Department. Mike stated that six farm workers were sprayed <br /> with round up power max 2. Stated that they had symptoms of vomiting, <br /> headaches, nausea, and shortness of breath. The incident occurred at 12355 W. <br /> HWY-12 Lodi, CA 95242. Mike stated that Lodi Fire received the call <br /> approximately around 2:00pm. Individuals were exposed to round up at <br /> approximately 1:50pm. Lodi Fire Department transported the six individuals <br /> sprayed with Round Up Power Max 2 to Lodi Memorial Hospital, arrival time was <br /> approximately 3:50pm. Discussed information with Muni Naidu lAssistant Directorl, <br /> Melissa Nissim jProgram Coordinator) and Elena Manzo ILead Senior REHSj. <br /> Based on discussion I contacted AG commissioner's office, talked to Jana <br /> Lebrupherie,AG Biologist, she stated she would give the information to Martin <br /> Brockman, Deputy AG Inspector. Received a call from Martin Brockman, gave him <br /> information regarding the incident and the contact information to Mike Alegre. <br /> 02/08/2021 This is a continuation of complaint received during the day. GAITAN <br /> Received a call from Mike Alegre, stated that all six individuals were showing <br /> minor symptoms of shortness of breath and nausea. Also, stated that all six of the <br /> individuals were currently going through decontamination using the hospital <br /> decontamination showers. Informed Mike that Martin Brockman from AG <br /> commissioner's office would be contacting him. <br /> Discussed updates with Elena and Melissa, based on discussion I drafted an <br /> email to inform Dr. Maggie Park, Director of San Joaquin County Public Health. <br /> Sent email to Dr. Park. <br /> Generated complaint. <br /> 5:30pm <br /> 02/09/2021 Wrote narrative and finished complaint. GAITAN <br /> 02/09/2021 Contacted Elena Manzo Lead Senior REHSj discussed complaint. Based on GAITAN <br /> discussion I contacted Mike Alegre, Battalion Chief for Lodi Fire Department. <br /> Asked if round up was released on to soil, he stated that he is uncertain if Round <br /> Up impacted soil. <br /> Contacted Martin Brockman, Deputy AG inspector for AG Commissioners office. <br /> Martin stated that AG biologist,Augustine Diaz, is going out to incident area today. <br /> He stated that he would have Augustine contact me once he inspects the farm. <br /> Updated Elena. <br /> 5105.rpt Page 1 of 2 <br />
