S A N :10 A Q U I N Environmental Health Department <br /> " U) -COUNTY— <br /> `Ci�7ss �' Greatness grows Time In: <br /> Time Out: <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: WASTE MANAGEMENT Date: 03/02/2021 <br /> Address: 2220 CHAPARRAL CT, LODI <br /> Owner/Operator: WASTE MANAGEMENT Telephone: (209)313-4817 <br /> Program Element: 2546-RELEASE/SPILL RESPONSE(EXCLUDING JOINT TEAM) Complaint#: C07SJMPWF <br /> Responsible Party: HENRY FONTES <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> A hose on a truck split causing the release of three gallons of coolant. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> NOTES <br /> OBSERVATIONS: EHD conducted a site visit on 3/2/2020. The release occurred on Chaparral Ct, Sylvan Way, and Lupine <br /> Ct. Absorbent saturated with coolant was observed on the ground toward the end of Chaparral Ct. No saturated absorbent <br /> was observed on Lupine Ct and Sylvan Way. No storm drains were observed impacted. The waste generated from the initial <br /> cleanup was put into one 55 gallon drum and taken to the 1333 E Turner Rd, Lodi, CA 95240 yard,for storage until disposal. <br /> The waste was labeled as hazardous waste. <br /> CORRECTIVE ACTIONS: Immediately complete cleanup of the rest of the saturated absorbent and return the street to <br /> conditions before the incident happened. Notify the EHD once final cleanup has been completed and provide photos.
