Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COJBAF7LM Site Location: 8010S.AIRPORT <br /> Activity Summary <br /> Activity Date Recorded by <br /> 02/09/2021 Muniappa Naidu jProgram Coordinatorl, Melissa Nissim jProgram Coordinators GAITAN <br /> and Elena Manzo ILead Senior REHSJ discussed complaint received by Sgt. <br /> Crutchfield, CHP. Stated that individuals were exposed by unknown substance at <br /> 8010 S. Airport Stockton, CA. Based on discussion I contacted Sgt. Crutchfield. <br /> He stated that a hazmat team is being put together consisting of Montezuma Fire, <br /> Stockton Fire, Lodi Fire, Sheriff's office, CHP, DA Investigators, and DOJ. He <br /> stated he could not disclose any additional information unless I was onsite. <br /> Discussed information with Muni and Melissa, based on discussion I left to go <br /> incident area. <br /> Met with Sgt. Crutchfield and Sgt. Sheeran, Stated that four employees were <br /> exposed to an unknown white powder concealed in a white envelope. Attached to <br /> the envelope was a letter that referenced white supremacy and civil war. Sgt. <br /> Crutchfield notified me that DOJ will not be arriving and USPS will be sending an <br /> inspector in to collect the white powder. Sgt. Crutchfield stated that the four <br /> individuals were exposed to the unknown white powder at approximately 10:54am. <br /> CHP responded and notified Montezuma Fire Department approximately 10:58am. <br /> Contacted Melissa updated her complaint, based on discussion I waited until <br /> USPS arrived on site. <br /> Spoke to E. Foley, USPS Inspector, she stated she would be sampling the <br /> unknown white powder and removing the unknown white powder from the site. <br /> Contacted Melissa discussed new information with her. Based on discussion I <br /> talked to Sgt. Sheeran, asked if anything was needed from EHD. He stated no, he <br /> gave me his business card and I gave him mine, I requested he contact me if the <br /> individual involved begin showing any symptoms. He stated he would. <br /> Updated Elena, Melissa, and Muni. Generated complaint, sent an email to Sgt. <br /> Sheeran, requested he contact me if individuals show any symptoms. Typed <br /> narrative. <br /> 02/12/2021 Generated photo log. GAITAN <br /> 02/12/2021 Contacted Sgt. Crutchfield, CHP. Discussed the exposed individuals, stated that GAITAN <br /> they showed no symptoms and that the USPS inspector determined the white <br /> powder to be a non-hazardous substance. <br /> 03/22/2021 Generated site map, emergency response record, complaint investigation report, GAITAN <br /> emails. Reviewed dailies based on review, I contacted Sgt Crutchfield, Stockton <br /> Sheriff. Left a voicemail requested any report from the USPS inspector that <br /> sampled the white powder. <br /> 05/11/2021 Reviewed complaint for closure. Called Sargent Crutchfield, 209-815-6511, to MANZO <br /> follow up on the white powder sampling analysis. Sargent Crutchfield, indicated <br /> that it was determined to be non-hazardous and all he is allowed to disclose that <br /> "you can eat it and it will not hurt you", he indicated that the case can be closed. <br /> Abated the complaint. <br /> 5105.rpt Page 2 of 2 <br />
