I will deliver a copy onsite when the work begins. I also
intend to be onsite at the end of each day to track how the work is progressing.
Fyi, the MFR form you are using is out of date. If you look on our website at
http:www.sjgov.org/ehd, on the right hand side towards the bottom there is a line that
says, "Requirements for a boring permit". Clicking on that will take you to a page with
three lines that end in 'click here". The third one says "To Complete a Master File
Record (MFR), click here". Clicking on that line on that page (sadly not this one) will
bring up a new Master File Record form. (It seems that if you hit control and click on the
click here line it may bring up the new mfr).
Let me know when your people intend to be in the field to begin work.
Johnny Yoakum
From: Anna Argyridou [mailto:aargyridou@gercorp com]
Sent: Monday, March 12, 2012 10:01 AM
To: John Yoakum [EH]
Subject: RE: Permit for Holt, CA
Hi Johnny,
I believe we are combining the work with our quarterly sampling. If we receive the permit today
we will plan on starting tomorrow. I will have Tom Petty, the staff geologist that will be out
there give you a call for more information.
3/12/2012
