OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: - 87 -product and 91 -product fill sump fuel alarms were not documented by the designated operator in section 3. - follow-up actions to 87 -product and 91 -product fill sump fuel alarms were not documented by the owner/operator in section 5. REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. ----------------------------------------------------------------------------------------------------------------------------------------------- OBSERVATION: Designated operator reports were not signed within the required time frame. - The 11/12/2020 designated operator report states that the owner/operator signed the report on 11/12/15. REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report, the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class ll. 303 CCR 2630(d), 2641(a) Failure to maintain piping to allow liquid to flow into sump (i.e. failure to remove test boots). OBSERVATION: The test boot on the 87 vent piping was observed not pulled back with the Schrader valve closed and pointed up. Any potential release from the primary containment into the secondary containment in the 87 vent piping does not flow into a collection sump. REGULATION GUIDANCE: All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the low point of the underground piping.
