The owner or operator shall prepare a contingency <br />plan designed to minimize hazards to human health or the environment <br />from fires, explosions, or any unplanned release of hazardous waste to <br />air, soil, or surface water and immediately implement that plan <br />whenever there is a fire, explosion, or release of hazardous waste <br />which could threaten human health or the environment. The contingency <br />plan must include: 1. Description of actions facility personnel will <br />take in response to fires, explosions, or any sudden or non -sudden <br />release of hazardous waste to air, soil or surface water at the <br />facility; 2. Description of arrangements made with local police <br />departments, fire departments, hospitals, contractors and State and <br />local emergency response teams to coordinate emergency services; 3. <br />List of names, addresses, and phone numbers (office/home/cell) of all <br />persons qualified to act as emergency coordinator.
