Site Health and Safety Plan <br />Countryside Market, Stockton, California March 21, 2014 <br />Evacuate other personnel at the Site to safe places until the Site Health and Safety <br />Coordinator determines that it is safe for work to resume. <br />The Site Health and Safety Coordinator is responsible for immediately notifying the Project <br />Manager, and preparing and submitting an Incident Report to the Health and Safety <br />Director within 24 hours, as well as notifying the employee's supervisor and Principal-in- <br />Charge. Incident Report forms are available in Appendix D for use in the field at the time of <br />the incident, but it must also be completed online within 24 hours at <br />http:/intranet.thesourcegroup.net. <br /> <br />9.5 Decontamination Procedures during an Emergency <br />Decontamination of an injured or exposed worker or during a Site emergency should be performed <br />only if decontamination does not interfere with essential treatment or evacuation. If a worker has <br />been injured or exposed and decontamination can be done, then wash, rinse, and/or cut off <br />protective clothing and equipment. <br />If a worker has been injured or exposed and cannot be decontaminated, then perform the following <br />tasks: <br />Wrap the victim in blankets, plastic, or rubber to reduce contamination of other personnel; <br />Alert emergency and offsite medical personnel to potential contamination; and <br />Have the Site Health and Safety Coordinator or other personnel familiar with the incident <br />and contaminants at the Site accompany the victim to the hospital. If possible, send a copy <br />of the appropriate Material Safety Data Sheets (MSDSs) with the victim. <br /> <br />9.6 Directions to the Hospital from the Site <br />The route and directions to the nearest local hospital from the Site are illustrated in Figure 3. <br /> <br />9.7 Spill Control <br />To minimize the potential for spills, proper handling of chemicals is important. The primary risk for <br />spills will come from investigative derived waste consisting of water generated from the <br />decontamination of equipment, and purge water generated from well sampling and well <br />development. Purge water and decontamination water will be stored onsite in 55-gallon drums, <br />therefore proper drum handling practices is crucial in the prevention of accidental spills. To <br />minimize the potential for spills to occur, workers will adhere to the hazard mitigator for drum <br />handling and to the requirements of 8 CCR 5192(j). Drum hazard mitigators are as follows: <br />Use only drums and containers that meet the appropriate DOT, OSHA, and EPA <br />regulations; <br />A <br />HASP 21MAR2014 <br /> 9-3 <br /> APEX
