Site Health and Safety Plan <br />Countryside Market, Stockton, California March 21, 2014 <br />"Threatened release" means a condition creating a substantial probability of harm, when the <br />probability and potential extent of harm make it reasonably necessary to take immediate action to <br />prevent, reduce, or mitigate damages to persons, property, or the environment. <br />"Hazardous material" means any material that, because of its quantity, concentration, or physical <br />or chemical characteristics, poses a significant present or threatened hazard to human health and <br />safety or to the environment, if released into the workplace or the environment. <br />In the event of a significant release or threatened release of hazardous materials, it must be <br />reported IMMEDIATELY to the Project Manager or Principal-in-Charge. They will handle the <br />appropriate reporting requirements. Be prepared to provide the Project Manager with the following <br />information as State notification requirements for a spill or threatened release include: <br />Identity of caller <br />Location, date and time of spill, release, or threatened release <br />Location of threatened or involved waterway or stormdrains <br />Substance, quantity involved, and isotope if necessary. <br />Chemical name (if known, it should be reported if the chemical if extremely hazardous) <br />Description of what happened. <br />The Project Manager should call the following: <br />California Office of Emergency Services (OES), California State Warning Center <br />800-852-7550 <br />Outside California call 916-845-8911 <br />If appropriate, 911 or Fire Department in the event of real or perceived immediate danger to <br />life or health or the environment. <br />If appropriate, California Highway Patrol for spills occurring on highways in the State of <br />California <br />Upon receiving a report concerning a spill, unauthorized release, or other accidental release <br />involving hazardous materials, the OES will immediately inform appropriate agencies. <br />Additional information on spill/release notification and reporting in California can be found on Apex- <br />SGI's intranet at https://apexcos.sharepoint.com/sites/HS/SitePages/Home.aspx under "Program <br />Documents" or at the following website: <br />http://www.oes.ca.qov/Operational/OESHome.nsf/PDF/SpilP/020Notificationc)/020Guide/$file/SpillNotif06.pdf. <br />A <br />HASP 21MAR2014 <br /> 9-5 <br /> APEX
