Gall Trucking Company and the former or current owner of the Equinoa Property. Did C. G. Gall Trucking Company transport and/or dispose of asbestos waste to or at any other site? If so, please provide the locations and dates of the transport or dis- posal. Please provide any information or knowledge you or your employees may have concerning the disposal of asbestos-bearing wastes at the Equinoa Property. Please provide copies of all documents related to asbestos disposal practices, including but not limited to: invoices, contracts, accounts receivable, trip tickets, weight cer- tificates. Provide the names of: a) the persons who prepared the responses to each of the above questions and, b) the persons who provided information for the preparation of the responses to the above questions. Your response to this request for information must be sent to EPA at the above address within fifteen (15) calendar days of your receipt of this letter. Under Section 104(e)(5) of CERCLA as amended, 42 U.S.C. 9604(e)(5), failure to comply with this re- quest may result in an Order requiring compliance or a civil ac- tion for appropriate relief, including penalties.
