MONITORING AND REPORTING PROGRAM ORDER NO R5-2015-0012-0021 <br />3 <br />BOULEVARD AUTOMOTIVE SERVICE <br />SAN JOAQUIN COUNTY <br />amount which shall not exceed five thousand dollars ($5,000) for each day in which the violation <br />occurs. <br />It is Hereby Ordered that the Discharger shall comply with the following Monitoring and <br />Reporting Program (MRP) requirements: <br />GENERAL REQUIREMENTS <br />All samples should be representative of the volume and nature of the discharge or matrix of <br />material sampled. The time, date, and location of each grab sample shall be recorded on <br />the legal chain of custody form. <br />GROUNDWATER MONITORING <br />As shown on the attached Figure 1, there are thirty-four (34) existing groundwater <br />monitoring wells (MW-1A, MW-1B, MW-2 through MW-9, MW-10 (A through E), MW-11 (A <br />through E), MW-12 through MW-24, DPE-1 and DPE-2. The groundwater monitoring and <br />reporting program (MRP) for the groundwater monitoring wells and any groundwater <br />monitoring wells installed subsequent to the issuance of this MRP, shall follow the schedule <br />below. Groundwater monitoring wells with free-phase petroleum product or visible sheen <br />shall be monitored, at a minimum, for product thickness and depth-to-water (DTVV). Sample <br />collection and analysis shall follow standard Environmental Protection Agency (EPA) <br />protocol. <br />For the purposes of the proposed full-scale implantation, the wells have been divided and <br />identified as background wells (MW-5 and MW-12), treatment zone wells (also referred to <br />as performance indicator wells; MW-1A, MW-3, MW-4 and MW-15), transition zone wells <br />(MW-9, MW-13 and the proposed shallow transition zone well), and compliance zone wells <br />(Chevron well MW-10 or proposed shallow monitoring well, Blvd. Auto well MW-7 and the <br />proposed and intermediate compliance well [Figures land 2]). All other wells are either <br />cross-gradient or are discretely-screened wells monitoring water quality and occurrence in <br />deeper portions of the saturated zone beneath the site; therefore, these wells will not be <br />used as an ISCO performance/compliance observation point, but will continue to be <br />monitored/sampled as part of the ongoing semi-annual groundwater monitoring program. <br />The groundwater monitoring wells shall be sampled according to the schedule in Tables 1A <br />and 1B and the samples analyzed by the methods in Table 2, as follows:
