Date <br />ACCESS AGREEMENT <br /> DEC 2 6 2fiii.3 <br />Groundwater Monitoring Well Installation <br />1151 Holly Drive, Tracy, California <br />(APN: 233-120-15) <br />I, Denzle Thompson, property owner of 1151 Holly Drive, Tracy California (Parcel 233- <br />120-15), do hereby grant permission to Advanced GeoEnvironmental, Inc. (AGE) on <br />behalf of the responsible party, Levand Family Trust and Levand Bright Family Trust, to <br />advance one (1) soil boring for the installation of a groundwater monitoring well under <br />San Joaquin County permit on the property located at 1151 Holly Drive, Tracy, California <br />(see attached figure). The well will be installed as part of an investigation of a leaking <br />underground storage tank (LUST) site at 3 East Eleventh Street, Tracy, California. The <br />proposed work will be performed under a directive from the Central Valley Regional Water <br />Quality Control Board. Well installation field work will be overseen by AGE staff. The <br />scope of work will include the following: <br />Installation of one (1) 8.25-inch diameter groundwater monitoring well on the <br />property located at 1151 Holly Drive, Tracy, California. The well will be installed to <br />a total depth of 20 feet below surface grade using a standard drilling rig. The <br />surface at the proposed well location will be finished with a traffic-rated well box. <br />Groundwater samples will be collected from the well quarterly for the first year and <br />semi-annually thereafter if necessary. <br />At the conclusion of the investigation, the well will be properly removed under San <br />Joaquin County permit and the surface will be returned to the original condition. <br />The well will be properly removed at any time during the investigation if the property <br />owner, Mr. Denzle Thompson, requests that it be removed for any reason including <br />development or sale of the property. <br />All expenses for the proposed well installation, site assessment activities, and periodic <br />monitoring shall be the sole responsibility of the Levand Family Trust and Levand Bright <br />Family Trust (Trusts) and shall be conducted under permit and in accordance with local <br />regulations. Reporting of all work at your property and for the investigation will be <br />forwarded to you the property owner or agent for the property owner. <br />AGE will provide a minimum notice of 48-hours prior to the initiation of any field activities. <br />All field activities will be conducted in a manner to minimize disruption of normal site <br />operations. <br />Si <br />j_//i4v) <br />mt.
