The site parcel at 3 East 1 1 th St; the parcel to the north <br />of the site at 1120 Holly Drive; and the parcel to the West of the site at 1101 Holly Drive. <br />Access agreement for the parcel located at 1101 Holly Drive; Our client owns the other 2 <br />properties. <br />Licensed Contractor Declaration page. <br />Site Mitigation Master File Record Information Form (MFR) <br />Site Plan <br />Check for the permit fees in the amount of $1,390.00. <br />It has come to my attention that you may require additional information to process the permit <br />applications. Attached is supplemental information for the permit package.
