Earth Systems

Southwest 1680 Illinois Ave. Suite 20
Perris, CA 92571
Ph: 951-928-9799
Fax: 951-928-9948
www.earthsystems.com
April 3, 2017 Proposal No.: SWP-17-058
MEDISTAR CORP
7670 Woodway Drive, Suite 160
Houston, Texas 77063
Attention: Mr. Warren W. Barnes III, SVP
Subject: Proposal for Preliminary Geotechnical Engineering Report
Projects: Proposed Stockton Rehab Hospital
North California Street & Magnolia Street
Stockton, San Joaquin County, California
Based upon the request for a proposal dated March 20, 2017 Earth Systems Southwest [Earth
Systems] is pleased to submit this proposal to provide geotechnical services for the above
referenced project. Since 1969, the Earth Systems group of companies has provided expert geo-
professional services to assist our clients through all aspects of geotechnical and geologic
engineering, environmental engineering, and construction related materials testing and
inspection services. Through our previous work with the California Office of Statewide Health
Planning and Development (OSHPD) related projects, our dedicated staff understands the diverse
geotechnical, geologic, and construction-related issues that may affect the proposed project, and
will work to develop practical and economical geotechnical engineering solutions through
innovative approaches.
We have prepared this proposal based on our understanding of preliminary project information
provided, via email, on March 20, 2017. We postulate that the proposed rehab hospital will be
developed under the regulation of OSHPD, the 2016 California Building Code [CBC] and California
Geological Survey [CGS] Note 48 requirements (2013) for geotechnical reports submitted to
OSHPD. As such, we have scoped our proposal to address additional levels of reporting required
by these agencies. Based upon the anticipated conditions, the following sections describe our
understanding of the project and our anticipated scope of services in order to meet the above
requirements.
This project will be managed by Mr. Kevin Paul, GE 2930, who will also be providing the
engineering evaluation and report preparation. Mr. Mark Spykerman, EG 1174, will also provide
the interpretation of the site geologic conditions and will be the principal in charge of this project.
PROJECT UNDERSTANDING
We anticipate the proposed project will include construction of a two story 50-bed rehab
hospital. Total square footage of the planned building is approximately 60,000 ft'. Conventional
shallow continuous and square pad foundations with slab-on-grade floors are anticipated.
