during grout installation will be pumped out of the excavation and containerized for proper
disposal.
Waste Removal
Following the well destruction process, all investigation derived waste will be removed and
properly disposed of. Documentation of the waste disposal will be included in a site closure
summary report.
Permitting
Permit number WP0037620 was issued on November 21, 2017 (Attachment B); we trust that no
additional permitting is needed.
Schedule and Reporting
The well destruction work will commence as soon as the driller is available. We will provide
EHD with at least 48-hours advance notice prior to beginning fieldwork. A summary of the well
destruction and site closure activities will be presented in a report, including a discussion of the
well destruction efficiency (grout volume used versus grout volume calculations). Copies of the
report will be submitted to both the appropriate County and State regulatory agencies.
If you have any questions or comments please contact Ground Zero at your convenience.
Respectfully,
Ground Zero Analysis, Inc.
Andrew Dorn, B.S.
