Michael <br />Please find enclosed the original construction permit, the abandonment workplan, master file and application. <br />Terrax obtained a Well Construction Permit from the San Joaquin County Environmental Health Department for <br />advancement of the soil borings. A copy of the permit is attached. The permit was issued on October 17, 2016 for the <br />advancement and construction of the soil vapor monitoring wells. <br />Soil vapor probes were constructed within soil borings B1, B2, B3, B4 and B6. No soil probes were constructed in soil <br />boring B5. Following completion of drilling, new 1 -inch diameter polyvinyl chloride (PVC) casing was inserted into the <br />borehole to the desired depth. New 1/4-inch diameter Nylaflow® nylon tubing with a 3/8-inch diameter polypropylene <br />filter at the terminal end was inserted into the borehole through the PVC casing to the desired sampling depth. The PVC <br />casing was used as a guide for the tubing to ensure that the desired sampling depth was achieved. No. #3 Monterey <br />sand was poured into the boring annulus through the PVC casing to form an approximately 1-foot long sand pack around <br />the polypropylene filter, at which time the PVC piping was withdrawn and approximately 1 foot of dry, granular <br />bentonite (bentonite #18) was placed atop the sand pack, poured through the PVC casing. The casing was then slowly <br />removed as hydrated bentonite was added until the borehole was backfilled with hydrated bentonite to the next probe <br />placement point, where the sand pack placement procedure was repeated, or was backfilled to approximately 2-3 <br />inches bgs to form a seal. The sampling end of the tubing was fitted with a valve and the probe was labelled for <br />identification.
