<br />OBSERVATIONS: Hand sink at dough prep room is not draining. It shall be immediately unpluged.
<br />CALCODE DESCRIPTION: The potable water supply shall be protected with a backflow or back siphonage protection device, as required
<br />by applicable plumbing codes. (114192) All plumbing and plumbing fixtures shall be installed in compliance with local plumbing
<br />ordinances, shall be maintained so as to prevent any contamination, and shall be kept clean, fully operative, and in good repair. Any hose

<br />OBSERVATIONS: Floor base in back room has peeled off wall (next to desk) and also at the front of the facility close to
<br />wood cabinet. In order to prevent roaches from entering and exiting through gaps, seal linoleum to wall. Correct,
<br />immediately.
<br />Noticed that a rodent gnawed floor base to the right of mop sink. Seal area and continue pest control.

<br />OBSERVATIONS: Ceramic base tiles at pizza assembly line has fallen off underneath hand sink. Install new coved ceramic
<br />tiles by 2 weeks.
<br />Linoleum floors are in disrepair. Several floor areas are now bare concrete. This is a repeat violation for several years.
<br />Sections at food prep and in walk in cooler shall be either completely replaced or patched up. Correct, immediately.
<br />CALCODE DESCRIPTION: The walls / ceilings shall have durable, smooth, nonabsorbent, light-colored, and washable surfaces. All floor
<br />surfaces, other than the customer service areas, shall be approved, smooth, durable and made of nonabsorbent material that is easily
<br />cleanable. Approved base coving shall be provided in all areas, except customer service areas and where food is stored in original
