4.1.3 "Strccck-by" Hazards <br />Hazard: Injuries can, and often, result when one becomes an unexpected <br />receptor of contact with another mass. These occurrences typically result from <br />the worker being struck by a dropped or collapsed mass or a moving piece of <br />equipment or vehicle. Hazards with machines and heavy equipment are created <br />when there are rotating, reciprocating, and transverse motions, or cutting, <br />punching, shearing and bending actions. The following are potential "struck -by" <br />hazards that may be realized on this project: <br />• Moving Vehicle <br />• Heavy Equipment Operations/ Pavement Cutting <br />• Excavation Cave -Ins <br />Control s -Specific <br />Worlc Vehicles/Vehicular Tra� <br />Engage the vehicle's warning light bar whenever planning to pull off or <br />exit the street. When stopped or parked, continue use of the light bar. <br />Employees/workers shall not exit the vehicle until they have successfully <br />pulled off of the street. In those instances where it is not possible to clear <br />the shoulder, workers shall exit the vehicle on the side opposite the <br />adjacent traffic flow. Employees will be required to wear fluorescent vests <br />and place safety cones at 10 -yard intervals for a minimum of 30 yards (if <br />achievable) from the left rear corner of the vehicle so they may be seen by <br />adjacent traffic. <br />All street closures, detours, lane closures, signs, lights and other traffic <br />control devices shall conform to the California Manual on Uniform Traffic <br />Control Devices (California MUTCD), <br />http://www.dot.ca.gov/hq/traffops/engineering/mutcd/. <br />Workers shall maintain a constant awareness of traffic <br />patterns/conditions throughout the duration of the field services. <br />Heavl�gicipnTent Operations <br />Heavy equipment shall be equipped with a backup alarm to warn <br />workers that the vehicles are moving in reverse. Personnel working in <br />proximity to operating equipment shall maintain a high degree of <br />awareness and remain out of harm's way of the moving portions of the <br />equipment. In addition, personnel shall refrain from wearing loose <br />jewel y or clothing, especially when in proximity to rotating tools. <br />Personnel shall also wear hard hats and fluorescent safety vests.
