Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />June 07, 2021 <br /> Facility Address: <br /> 1721 S CHEROKEE LN, LODI <br /> Facility Name: <br /> QUIK STOP MARKET #152 <br />Violations <br /> 1055 □ V □ R □ COSUnlisted Abandonment/Illegal Disposal/Unauthorized Treatment - Local Ordinance See below <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 4 HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator Identification page. <br />OBSERVATION: The Business Owner/Operator Identification form was not submitted on time for 2019. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: This was corrected with the 2020 and 2021 submittals. <br />This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The hazardous materials inventory was submitted late for 2019. <br />REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br />the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br />States Code. An annual electronic submittal satisfies these requirements. The annual business plan shall be <br />submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br />previous year. <br />CORRECTIVE ACTION: The violation was corrected with the 2020 and 2021 submittals. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map was submitted late for 2019 and the most current site map is incomplete/inaccurate. <br />-The site map is missing loading areas. <br />-The site map does not list the chemical name for the underground storage tanks. <br />-The map inaccurately indicates that there is a fire extinguisher outside the store next to the emergency pump <br />shut-off. A fire extinguisher was not observed at this location. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br />FA0000591 PR0519949 SC001 06/07/2021 <br />EHD 19-01 Rev. 9/16/2020 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
