SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR WELL INSTALLATIONS <br />Site Control and Safety Procedures <br />May 4, 2017 <br />placed into effect until it has been signed by the Qualified PTW Approver and a representative <br />of each subcontractor as acknowledgement of the work to be performed that day. If the <br />scope of work includes any high-risk tasks, the Qualified PTW Approver must document that the <br />CEMC Project Manager has been notified and has given their acceptance before the PTW <br />becomes valid. Copies of the completed PTW should be provided to each field team. <br />Personal Protective Equipment (PPEJ Assessment <br />A PPE Assessment must be completed and documented during the pre-planning phase of the <br />project. The PPE Assessment determines additional PPE beyond the minimum standard Level D <br />that will be required for the project based on the anticipated tasks and activities on site. Any <br />additional PPE requirements identified by the assessment must be specified in the JSA for the <br />relevant task to which they apply. The completed PPE Assessment form must be signed by the <br />Stantec Project Manager and included in this HASP as Attachment 6. Refer to the CEMC OE <br />Required Personal Protective Equipment document for additional information. <br />The minimum standard PPE at the site is Level D and proper work attire, which includes a hard <br />hat, safety glasses with side shields, safety-toe boots, hi-viz leather/work gloves (specific glove <br />type to be determined by the PPE assessment), hi-viz chemical resistant gloves when handling <br />chemical or hazardous substances, hi-viz reflective safety vest, long sleeve shirt, and pants. <br />Clothing that is frayed, torn, excessively baggy, or has a hood or drawstring shall not be worn. <br />Pre-Entry Briefing <br />All on-site workers and visitors will receive a pre-entry briefing prior to accessing work areas of <br />the site. The briefing will include reviewing contents of the HASP, signing the Acknowledgement <br />and Agreement Form, and completing the Site Health and Safety Quiz (Attachment 9). <br />Completed quizzes will be kept with the project files. The briefing for visitors may be abbreviated <br />to be fit-for-purpose based on the intent of the visit. <br />Preventing Serious Injury & Fatalities (PSI&F) Field Guide <br />CEMC developed the PSI&F Field Guide as a quick reference to help personnel involved in high- <br />risk activities to identify and control the significant potential hazards unique to each activity and <br />job. This field guide supplements but does not replace existing permitting procedures and safe <br />work practices. It should be used in conjunction with the PPE Assessment, JSAs, and other <br />hazard assessment procedures to comprehensively assess each job. The PSI&F Field Guide is <br />included as Attachment 2 and applies to work involving: <br />Confined Space <br />Electrical Work <br />Excavation <br />Hot Work <br />Lifting & Rigging <br />Marine Work <br />Motor Vehicles <br />Work at Heights <br />Other high-risk activities <br />Public Questions and Press <br />Questions about the site posed by neighbor(s), the press, or other interested parties will be <br />directed to the CEMC Project Manager. <br />40
