SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR WELL INSTALLATIONS <br />Site Control and Safety Procedures <br />May 4, 2017 <br />6.0 Site Control and Safety Procedures <br />Procedures described in this section are intended to aid Stantec personnel in mitigating site risks <br />and hazards. <br />Daily Tailgate Meeting (DTMI <br />A DTM will be held on-site (and documented) each morning and afternoon involving all staff <br />and subcontractors on-site to review the scope of work, discuss and address potential hazards, <br />and answer any relevant questions before work begins. The topics of the meeting will include, <br />at a minimum, general health and safety procedures, reviewing health and safety policies, and <br />reviewing the JHA and JSA(s) for the tasks to be conducted. Additional safety meetings may be <br />conducted if the scope of work changes during the day, or if other health and safety issues are <br />identified. The DTM form is located in Attachment 7. <br />Driving <br />Review the Stantec Safe Driving Procedures provided on-site. <br />Utilize the Journey Hazard Assessment Card to identify potential driving/journey/traffic <br />hazards before each trip. Copies of the Journey Hazard Assessment Card are located <br />on-site. <br />Utilize the Daily Vehicle Checklist at least once a day for each vehicle driven for Stantec <br />business to identify potential vehicle issues/hazards. Copies of the Daily Vehicle <br />Inspection Checklist are located on-site. <br />Have each team member who will travel to/from the site review the site-specific Journey <br />Management Plan (JMP) before traveling to identify routes of travel and potential <br />driving/journey/traffic hazards. JMP(s) will be kept with each traveling employee <br />throughout the entire course of travel. <br />Before a driver enters a vehicle, Get Out and Look (GOAL). Conduct a 360 degree walk- <br />around of the vehicle to look for bystanders, traffic, obstacles, damage, and leaks in <br />order to reduce the risk of striking unknown objects in the environment. <br />When a driver leaves a vehicle, the engine shall be turned off, the transmission placed in <br />park (placed in first gear or reverse for manual transmission), the parking brake set, the <br />keys removed from the ignition and all doors closed. Exception to this is for operational <br />needs/power take off (e.g., vacuum trucks, drill rigs, or other similar vehicles). Vehicles <br />that fall into this exception category must have the parking brake set and wheels <br />chocked. <br />A Stantec Vehicle Collision Kit will be kept in every vehicle used for Stantec project work. <br />A copy of the Stantec Vehicle Collision Kit is located on site. <br />CEMC owned and/or leased vehicles will only be driven by Chevron employees. <br />Smoking is not permitted in Chevron owned, leased or rented vehicles. <br />Driving Improvement Monitors (DIMs) are required for all heavy vehicles dedicated 100% <br />to CEMC project use for more than three months. <br />Emergency Situations and Vehicle Breakdowns <br />If the vehicle becomes disabled, it shall be parked with all wheels off the main <br />roadway whenever possible. <br />When exiting the vehicle, personnel shall not place themselves between <br />oncoming traffic and the disabled vehicle. <br />If the vehicle becomes disabled within 500 feet of a curve, crest of a hill, or other <br />visual obstruction, the driver shall place warning devices such as reflective <br />36
