SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR WELL INSTALLATIONS <br />Emergency Response <br />May 4, 2017 <br />Date he/she is able to return to work, regular or light duty <br />Date he/she is to return to doctor for follow-up appointment, if necessary <br />Signature and address of doctor <br />The SHSO is responsible for notifying the Stantec Project Manager. The Stantec Project <br />Manager will initiate appropriate incident reporting procedures as described in Section <br />1.7. <br />If the injured employee is unable to return to the job site the same day, the employee who <br />transported him will bring this information back to the job site and report it to the SHSO who will <br />then communicate it to the Stantec Project Manager. The Stantec Project Manager will initiate <br />appropriate incident reporting procedures as described in Section 1.7. <br />1.5.2 Injury or Illness Requiring Hospital Service WITH Ambulance Service <br />Injuries or illnesses requiring hospital service with ambulance services include severe head <br />injuries, amputations, heart attacks, heat stroke, etc. The following procedures will be taken <br />immediately: <br />Call 911 or the emergency contact number listed in Section 1.1 for ambulance service. <br />Notify the SHSO. <br />Administer First Aid until ambulance service arrives. <br />One designated representative will accompany the injured employee to the medical <br />facility and remain at the facility until full diagnosis and other relevant information is <br />obtained. <br />If the injured employee is able to return to the job site the same day, he/she will bring <br />with him/her a statement from the doctor containing such information as: <br />Date <br />Employee's name <br />Diagnosis <br />Date he/she is able to return to work, regular or light duty <br />Date he/she is to return to doctor for follow-up appointment, if necessary <br />Signature and address of doctor <br />The SHSO is responsible for notifying the Stantec Project Manager. The Stantec Project <br />Manager will initiate appropriate incident reporting procedures as described in Section <br />1.7. <br />If the injured employee is unable to return to the job site the same day, the employee who <br />transported him will bring this information back to the job site and report it to the SHSO who will <br />then communicate it to the Stantec Project Manager.
