AM. eScreenLogic <br />Environmental Solutions <br />December 13, 2018 <br />SITE HEALTH and SAFETY PLAN <br />General Site Information <br />Site Address: 730 Channel Street, Stockton, CA 95202 <br />Contact Person: Art Feagles <br />Phone Number: Office (209-373-2803) / Cell (707-280-065SA <br />Description of Activity <br />Purpose of Activity: Install up to three (3) soil borings to approximate depth of 25 feet <br />Type of Site: Commercial building <br />Brief Description of Proposed Activities: eScreenLogic, along with California-licensed C57 driller, will <br />mobilize to the site to install up to three (3)pproximate 2.5-inch diameter borings using a direct-push <br />technology (DPT) rig. The site will have an underground service alert (USA North) requested for <br />subsurface utilities prior to drilling activities. The borings will be hand augured (5 feet deep) to <br />additionally_clear potential subsurface utilities. All site workers are required to have been trained and <br />annually certified with respect to hazardous waste operations. <br />Investigation Derived Material Disposal: The soil cuttings will be placed within 55-gallon drums and <br />stored on-site as "non-hazardous waste" until the representative sample is characterized at a certified <br />analytical laboratory allowing the material to be recycled or disposed of accordingly. <br />Potential Health and Safety Hazards <br />Anticipated Physical Hazards: Noise, falls/trips/slipping, heavy equipment, physical trauma/iniury <br />Chemical Hazards: <br />Chemical TLV (ppm) IDLH (ppm) LEL (%) <br />1 <br />Health <br />Benzene 10 500 1.2 Carcinogen <br />Toluene 50 500 1.1 Toxicity <br />Ethyl Benzene 100 800 0.8 Toxicity <br />Xylenes 100 900 0.9 Toxicity <br />TLV = Threshold Limit Value <br />IDLH = Immediately Dangerous to Life and Health <br />LEL = Lower Explosive Limit <br />Special Precautions: Values above are taken from the National Institute for Occupational Safety and <br />Health (NIOSH). A PID will be used to screen the breathing zone. If a sustained reading of 5 ppm is <br />observed, the on-site Safety Officer will halt operations until the breathing zone levels are back below <br />the PID threshold. <br />Personal Protection: Level D (hardhat, steel-toes boots, hearing protection, safety glasses, clothing <br />protection, high visibility vest) with PID in the breathing zone.
