The following is an itemized list of hazardous materials violations that have not been <br />addressed for SHELLPRO INC as of August 24, 2021. <br />Open violations from September 29, 2020 inspection <br />Violation #5 - Failed to electronically submit hazardous materials inventory for all reportable materials on <br />site. <br />The hazardous materials inventory has not been submitted annually since 2017 and 562 cubic feet of compressed <br />oxygen, 292 cubic feet of acetylene, 60 gallons of gasoline, and one 89 gallon diesel tank were found and have not <br />been reported. Additionally, the Hazardous Materials Inventory forms for the following material is not <br />complete/accurate: <br />Propane — storage temperature is wrongly identified as "below ambient' vs. "ambient'. <br />Waste oil — the annual waste amount element has not been completed. <br />This information must be current and submitted as part of the business plan. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br />submit to the EHD for approval. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #6 - Failed to electronically submit a site map with all required content. <br />The site map has not been submitted annually since 2017 and the 2017 site map submittal does not show/address: <br />1. Internal roads (none). <br />2. Storm and sewer drains are not adequately. <br />3. The storage and handling areas for compressed oxygen, acetylene, diesel, and waste oil are not shown. <br />4. Emergency propane shutoff is not shown. <br />5. Some codes used on the map are not defined. <br />6. The topographical information is not accurate; two additional silos, the new shop, the new warehouse, and the <br />40 ft. storage containers are not shown. <br />A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Page 1 of 3 <br />
