SAN J 0 A Q U 1 N Environmental Health Department <br /> U ; i y 1 Iii <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> SUBWAY SANDWICHES&SALADS#1 1186 N MAIN ST, MANTECA June 03, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Employee training records were not provided for review. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Conducted in-office Hazardous Materials Business Plans (HMBP) inspection. The food inspector, Gehane <br /> Fahmy , performed an on-site inspection for the CO2 tank on 2/16/2021. <br /> There were 7 violations found during the in-office inspection. complete and submit a copy of the Return to <br /> Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or <br /> will be taken for each violation, and any supporting paperwork, by 7/3/2021. Violation number#5 says corrected <br /> on site but this was done by mistake and the system will not let me edit this. Violation #5 still needs to be <br /> corrected. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
