The facility information section was <br /> last submitted late on 3/22/2021. The inventory and plans section were last submitted on 4/13/2018. The inventory <br /> and plans section have not been submitted for the years of 2019, 2020, and 2021. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Employee training records were not provided for review. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Conducted in-office Hazardous Materials Business Plans (HMBP) inspection. The food inspector, Gehane <br /> Fahmy ,, performed an on-site inspection for the CO2 tank on 1/27/2021. <br /> There were 7 violations found during the in-office inspection. complete and submit a copy of the Return to <br /> Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or <br /> will be taken for each violation, and any supporting paperwork, by 7/3/2021 <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
