Fremont Ave. Sunnyvale, CA Received call from <br /> Elena Manzo 11-ead Senior REHSj discussed complaint. Based on discussion I left <br /> a voicemail to Santa Clara County EHD, county where RP is located; (408) <br /> 918-3400. 1 also sent an email to Santa Clary County EHD; <br /> deh.hmcd@deh.sccgov.org. <br /> Contaced Elena, updated her on complaint. Based on conversation, I received <br /> additional phone numbers to contact. Contacted Santa Clara County Hazardous <br /> materials compliance division; (408) 299-7300. Left a voice mail. Contacted Vector <br /> Control Department for Santa Clara County; (408) 9184770 Spoke to front desk <br /> staff; she stated that the correct number to call would be (408) 918-3400. She <br /> stated that most inspectors work from home and explained that they have <br /> messages forward to them by front desk staff. Sent email to Elena updated her on <br /> complaint. Received call discussed complaint, stated I would update her once I <br /> receive a call from Santa Clara County. <br /> 11/25/2020 Discussed complaint with Garrett Backus Lead Senior REHSj. GAITAN <br /> 11/25/2020 Received voicemail from Santa Clara County EHD, Mathew Burge, Supervising GAITAN <br /> Haz Mat Specialist. Stated that he will be sending an email out to me. Received an <br /> email, stated that the city of Sunnyvale has their own CUPA and gave me the <br /> contact information for Rick Miller, senior hazardous materials specialist. Sent <br /> email to Rick Miller, requested information on De Anza Properties in regards to <br /> inspections/violations. <br /> 11/30/2020 Received email response from Rick Miller, Senior HazMat Inspector. Stated that GAITAN <br /> the De Anza Properties is not listed as a HazMat Facility. Requested if any <br /> evidence were provided to suggest illegal disposal of soil impacted with diesel. <br /> Forward email to Elena Manzo 11-ead Senior REHSj. <br /> 12/01/2020 Received email response from Elena Manzo 11-ead Senior REHSj. Based on email GAITAN <br /> reached out to Rick Miller, Senior HazMat Inspector. Sent email with complaint <br /> information to his email. Left voicemail. <br /> 12/03/2020 Received call from Rick Miller, Senior HazMat Inspector. Discussed complaint with GAITAN <br /> Rick, based on discussion it was determined that Sunnyvale CUPA will not be <br /> looking into the complaint due to lack of evidence and due to De Anza Properties <br /> not being a Hazardous materials/waste generator. Sent email to Elena Manzo <br /> 11-ead Senior REHSj updating heron conversation. <br /> 12/04/2020 Generated and printed complaint investigation form, site maps, narrative, emails.
