The facility's maintenance <br /> manager, Jerry Jastrem, was switching pumps for the PLR-2 recirculator located <br /> inside the engine room. Jerry turned off <br /> the pump supply and discharge valve for pump AP4 before switching to pump <br /> AP3. After turning pump AP3 back on, Jerry <br /> noticed a slight ammonia odor after approximately 20 minutes. Jerry used a sulfur <br /> stick to find the source of the leak. Jerry <br /> discovered the release was from the gland packing for the pump supply valve. He <br /> tightened the valve and stopped the <br /> release and the system returned to normal operation. The facility reported that the <br /> amount was too small to trigger the <br /> ammonia detection alarm. There are two ammonia detectors inside the engine <br /> room. The facility contractor is working on <br /> replacing the gland packing. There was no exposure or evacuation due to the <br /> release. The incident investigation is in <br /> progress. The San Joaquin County Environmental Health Department was the <br /> only agency notified. Facility thinks the amount too small to report to CAI OES, I <br /> discussed to follow their Emergency Action Plan. <br /> Vicky Vang/Trainee and I responded to the release 9:30am and arrived on-site at <br /> 10am. We met with Micheal and Jerry. I inspected the system and took photos. <br /> The system was not leaking at the time of the arrival and was in operation. <br /> Confirmed all the information received above. Left the site around 10:30am. <br /> OIR and inspection photos were emailed to Micheal, Jerry, and the Cooperate <br /> Compliance Manager, Juliana Chamber. <br /> I recommended reporting the incident to Cal OES since it is an accidental release <br /> and shared Title 19 CCR definitions of an accidental release. <br /> Asked the facility to submit the following items within 30 days: <br /> -Incident investigation report <br /> -Procedures for switching the PLR-2 pumps <br /> -Maintenance schedule/manufacturer recommendations/industry . <br /> Completed Prop 65, Emergency Response Records, Photo Log, and narratives. <br /> 5105.rpt Page 1 of 2 <br />
