%/n:�T Lineage <br /> II Form <br /> Weather Conditions, if Known: clear 48 degrees <br /> Number of Serious Injuries: 0 Number of Fatalities: 0 <br /> Environmental Damage: None Off-site Responders Notified, if Known: None <br /> Estimate of Property/Product Damage: None <br /> Team Members Involved: <br /> • Jerry Jastrem—Facility Maintenance Manager(FMM) <br /> • Ricky Ellington -APCCO Technician (3rd party refrigeration contractor) <br /> • Danielle Bockman - Facility Quality Control Supervisor <br /> • Michael Fritch—General Manager <br /> Incident Timeline: <br /> • 2:51AM—FMM Jerry Jastrem (JJ) received a call from 1st Security and Sound of a zone 50, which is <br /> temperature deviation. <br /> • 4:OOAM—FMM,JJ, arrived on site and proceeded into the engine room to verify system operations. <br /> The freezer System (Pressure Recirculator(PR)-2,Ammonia Pump(AP)-4,) was found to be not <br /> running. <br /> • 4:10AM—After JJ,verified pump motor failed.JJ started the process of switching AP-4 over to AP-3. At <br /> that point,JJ opened the liquid feed for AP-3 and discharge for AP-3.JJ then closed the liquid feed for <br /> AP-4 and closed the discharge for AP-4.After closing the discharge valve for AP-4,JJ started AP-3. <br /> • 4:40AM—JJ then verified the system operations after starting AP-3. <br /> • S:OOAM—JJ noticed a light smell of Ammonia and began troubleshooting. Using a sulfur stick it and <br /> personal detector it was determined that the gland packing nut was leaking by on the liquid feed for <br /> AP-4 valve number PR-2,33. JJ then re-tightened the valve (PR-2,33)and the gland packing nut and <br /> was able to stop leak. <br /> • 5:30AM—FMM notified GM <br /> • 5:40AM-JJ determined that there was no longer an ammonia leak. Ammonia 3'd party contractor <br /> (APCCO) was then called to come and inspect and verify that the leak had been repaired/stopped. <br /> • 5:45AM— FMM notified Regional Maintenance Manager(RMM) <br /> • 6:20AM—GM notified Regional Vice-President(RVP) of Operations <br /> • 6:31AM—GM notified Regional Compliance Manager(RCM). <br /> • 7:OOAM-APCCO arrived on-site, Ricky Ellington (technician) to determine that the leak was stopped <br /> but that the seal on the gland packing nut needs to be replaced. <br /> • 8:20AM—GM and FMM notified CUPA(Haza Saeed). <br /> Actions which helped minimize the result of the Incident: <br /> • Trained on-site FMM that had the ability to respond as quick as possible, including troubleshooting <br /> (i.e. eating the valve and tightening the gland packing nut). <br /> • Preventive Maintenance (PM) schedule includes annual valve exerting as per RAGAGEP (IIAR-6). <br /> Actions which could have helped minimize the results of the incident: <br /> None. <br /> Page 2 of 5 <br />
