THE
RELEASE WAS STOPPED AND THE SYSTEM RETURNED TO NORMAL OPERATION.
OVERALL INSPECTION NOTES AND COMMENTS
NOTES
An unknown amount of anhydrous ammonia released at approximately 5am this morning. The facility's maintenance
manager, Jerry Jastrem, was switching pumps for the PLR-2 recirculator located inside the engine room. Jerry turned off
the pump supply and discharge valve for pump AP4 before switching to pump AP3. After turning pump AP3 back on. Jerry
noticed a slight ammonia odor after approximately 20 minutes. Jerry used a sulfur stick to find the source of the leak. Jerry
discovered the release was from the gland packing for the pump supply valve. He tightened the valve and stopped the
release and the system returned to normal operation. The facility reported that the amount was too small to trigger the
ammonia detection alarm. There are two ammonia detectors inside the engine room. The facility contractor is working on
replacing the gland packing. There was no exposure or evacuation due to the release. The incident investigation is in
progress. The San Joaquin County Environmental Health Department was the only agency notified.
Submit the following items within 30 days:
-Incident investigation report
-Procedures for switching the PLR-2 pumps
-Maintenance schedule/manufacturer recommendations/industry standards for replacing the packing gland for the pump
supply valve
-Emergency action plan for reporting releases to CalOES
The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and
Safety Code.If a reinspection is required, fees will be assessed at the current hourly rate.
Emailed To Jerry Jastrem and Michael Fritch Name and Title: Maint Manger&GM
EH Specialist: HAZA SAEED Phone: (209)953-6212
FAD019734 C00053658 SC544 04/09/2021
EHD 25-01 Rev.01/07/2021 Page 1 of 1 Complaint Inspection Report
1868 E.
