The site map is incomplete and missing the <br /> following elements: <br /> -loading areas <br /> -internal roads <br /> -adjacent streets <br /> -storm/sewer drains: 1 storm drain was observed in the southwest corner of the parking lot near the underground <br /> storage tanks <br /> -evacuation staging area <br /> -hazardous materials storage areas for gasoline, carbon dioxide, carwash chemicals, and hazardous waste. <br /> -emergency response equipment:fire extinguishers were observed in the dispenser island, behind the cashier, in <br /> the storage room, and near the restrooms. The map indicates there is a fire extinguisher near the tank vent sump, <br /> however, a fire extinguisher was not observed in this area. <br /> -emergency shut-offs:there were 2 emergency shut-offs observed in front of the store outside on each end <br /> -access/exit points <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: <br /> -the emergency response plan was submitted late in 2019 and 2020. <br /> -the emergency response plan is missing the San Joaquin County local unified program agency(UPA)phone <br /> number(209-468-3420) <br /> REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> FA0021617 PR0537824 SCO01 06/21/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 3 of 5 Hazardous Materials Program OIR <br /> 1868 E.
