Harper, MHA, FACHE <br />Senior Vice President/Area Manager <br />Central Valley Service Area <br />Re: Stockton Medical Office, Hazardous Waste Generator Inspection Response <br />Dear Ms. Baker, <br />The purpose of this letter is to respond to the Hazardous Waste Inspection conducted at Kaiser Permanente <br />located at 7373 West Lane, Stockton, CA 95210 on 6/22/2021 for violations of non-compliance of the <br />following: <br />Ref #118: CCR 66265.16 - Failed to maintain complete hazardous waste training records. <br />Response: The departments completed refresher training on the hazardous waste program with focus <br />on container operations and labeling. <br />Attachment: Training sign -In Sheets (EVS, Engineering, Medicine A, Pharmacyl, Women's Health, ASU]. <br />Ref #303: CCR 66265.33 Failed to maintain all communication or alarm systems, spill control, or <br />decontamination equipment. <br />Response: The Engineering department cleaned up the liquid in the secondary containment per spill <br />response actions and place the clean-up waste into the Hazardous waste container. <br />Attachment: Training sign -in sheets for spill response training and photos of cleaned containment area. <br />Ref #403: CCR 66265.173 Failed to keep hazardous waste containers closed except when adding or <br />removing waste. <br />Response: The departments completed refresher training on the hazardous waste program with focus <br />on container management (keeping containers closed when not in use) and labeling (how to fully <br />complete the labels). <br />Attachment: Training sign -In Sheets (Engineering, Medicine A, Pharmacyl, Women's Health] and photos <br />showing how departments keep the lid closed when not using the waste containers. <br />Ref #601: CCR 66262.34 (a) - Stored hazardous waste on site longer than 90 days without a permit or <br />authorization. <br />Response: The EVS department manager had overdue waste containers picked up by waste hauler.
