COMPLAINANT STATES THEY ALL <br />ORDERED A CHEESE BURGER THAT COMES WITH BUN, PATTY, CHEESE, LETTUCE, TOMATO AND SPECIAL SAUCE, AND THEY <br />CONSUMED FOOD WHEN THEY GOT IT. COMPLAINANT ALLEGES THAT ABOUT 20 MINUTES LATER, THEY ALL STARTED TO FEEL ILL. <br />COMPLAINANT BEGAN EXPERIENCING SEVERE STOMACH PAINS, DIZZINESS, NAUSEA, TROUBLE BREATHING, SWEATING AND CHILLS. <br />COMPLAINANT WAS GOING TO GIVE LEFT OVER HAMBURGER TO DOG BUT DID NOT, HOWEVER THEY OBSERVED WHAT APPEARED TO <br />BE MAGGOTS IN BURGER. ACEVEDO <br />Upon arrival, I spoke with the manager on site in regards to the nature of the <br />complaint. The manager stated that no one has complained to them from their <br />knowledge and that no one has called stating that they were sick due to the items <br />ate from the facility or that they had found maggots in their food. I asked the <br />manager if anyone on site as been ill, left early from work due to sicknesses or <br />has left the country within the last 30 days. The manager stated that a majority of <br />their staff have not been arriving to work but not due to illness issues and no one <br />has left the country to their knowledge. I observed that the burgers on site are kept <br />frozen until placed on the grill to be cooked, and there are no flies on site at this <br />time. The manager also stated that the facility is cleaned by them and routine <br />maintenance for their ventilation hood filters and other equipment on site are done <br />by contractors. The manager stated that there is a homeless issue, in which the <br />homeless do enter into the facility to attempt to gain free food or other items from <br />the operators or customers on site. <br />Official inspection report emailed to ly2870@yahoo.com. <br />Official inspection report discussed with Kunthen Ly, signature not obtained at this <br />time. <br />06/02/2021 Called complainant with the number provided on the complaint. However, no one <br />was able to answer at this time and a voice mail was left with my contact <br />information for the complainant to call me when available. <br />I was able to receive a call back from the complainant later on in the day. I <br />explained my observations while conducting my inspection on site.
