The most recent financial responsibility <br /> documents submitted in CERS are dated 10/9/17. Current financial responsibility documents are required to be <br /> submitted annually. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a Class II violation. <br /> 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. <br /> The monitoring plan is not current and/or not approved by the EHD. <br /> -The monitoring plans for all four tanks do not include a second person having responsibility. <br /> -The monitoring plans for the diesel, 91, and 89 tanks incorrectly list the leak detector model as a LD2000. The leak <br /> detectors on site are 99LD2000. <br /> -The monitoring plan correctly states 'no'for tank integrity testing, but also states the testing is being conducted <br /> annually. <br /> -The monitoring plan for the 91 tank left UDC sensor manufacturer blank. <br /> -The monitoring plan for the 89 tank left UDC construction blank. <br /> The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log <br /> into CERS, make the necessary changes, and submit for review by the EHD. <br /> This is a repeat violation, Class II. <br /> 115 CCR 2715(a)(1)(B), 2715(a)(3)"Designated Underground Storage Tank Operator Identification Form" not submitted <br /> within 30 days. <br /> Paul Goodwin is performing designated operator inspections at this facility and notification was not provided to the <br /> EHD within 30 days of the change. Mr. Goodwin is not on the list of designated operators submitted by the facility. <br /> Any changes shall be reported to the EHD within 30 days.
