SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> LOCKEFORD AUTO 1 12962 BLOSSOM CT, LOCKEFORD June 25, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION 1: The Hazardous Materials Inventory forms for motor oil,waste ethylene glycol,waste motor oil, <br /> and ethylene glycol were not updated with the current federal hazard categories. <br /> REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br /> the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code.An annual electronic submittal satisfies these requirements.The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The map was missing the following items: <br /> -Loading Areas <br /> -Internal Roads <br /> -Emergency Shutoffs <br /> -Hazardous material handling and storage areas for all chemicals listed in inventory <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. <br /> The 2018 facility, inventory, and plan elements were submitted late on 3/1/2018. <br /> A 2019 submittal was not made for the facility, inventory, and plan elements. <br /> The 2020 facility, inventory, and plan elements were submitted late on 2/20/2020. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> FA0016348 PR0524660 SCO01 06/25/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
