Ethylene Glycol—the days on site element has not been completed. <br /> 5. Nitrogen gas—the size of the largest container, average daily amount, storage pressure, and storage <br /> temperature elements have not been completed. <br /> 6. 10w30—the average daily amount has not been completed. <br /> 7. Zep mudslide—the chemical name/hazardous components have not been identified;the hazardous <br /> materials type, size of the largest container, and average daily amount elements have not been <br /> completed. <br /> 8. Zep truck wash—the chemical name/hazardous components have not been identified and the <br /> hazardous materials type has not been completed. <br /> • Violation #6,the site map does not show or address the following: <br /> 1. Spill control equipment. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> Violations 5, 6, and 10 remain open. <br /> Be advised,the violation comply by date has passed, consequently all time spent attempting to obtain compliance may <br /> be billed at$152 an hour. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
