S0 JOA U 1 Environmental Health Department <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> STANFORD CROSSING SDPS/WWPS 783-785 SPARTAN WAY, LATHROP June 23, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION:A site map was not complete.The site map is missing a loading area, storm drain, locations, <br /> emergency shut offs, and emergency response equipment locations. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 8 HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br /> OBSERVATION: The training program was not complete.The training program has a comment that says"Provided <br /> else where in CERS"under the"Facility Information"section. Change the comment to say provided else where in <br /> CERS under the"Emergency Response and training plan"section. <br /> REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, <br /> shall include familiarity with the facility's Emergency response plans and procedures in the event of a release or <br /> threatened release of a hazardous material.Training shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> These training programs may take into consideration the position of each employee. <br /> CORRECTIVE ACTION: Immediately prepare or update the facility's training program, log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0025981 PR0545942 SCO01 06/23/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
