S0 JOA U 1 Environmental Health Department Underground Storage Tank Program Inspection Report Facility Name: Facility Address: Date: ORLANDOS 1 18754 E HWY 26 , LINDEN July 26, 2021 SUMMARY OF VIOLATIONS (CLASS I,CLASS II,or MINOR-Notice to Comply) Item# Remarks 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. 1)OBSERVATION:The designated operator failed to check and note the correct test dates for the overfill prevention inspection on the 8/23/2020 designated operator inspection report.According to EHD records,the overfill prevention inspection was conducted on 9/28/2018. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ---------------------------------------------------- 2)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -On the 2/1/2021 report,the designated operator documented visual inspections of the sumps but left the question regarding sump sensor locations blank. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 7/6/2021 lists the incorrect initial training date for Jerry Sinner. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a Class II violation.
