LEAP93 SUBMISSION INSTRUCTIONS ' SUBMMAL REQUMWS • F-"entry must comply with the following requirements to be considered for evaluation. Entry materials become the property of SWANA. please keep a copy of your entry for your records. 1. Each entry shall consist of one (1)3-ring binder suitable for SA z 11 pages, which shall contain all of the required application materials. The total number of pages including illustrations,pictures, etc. is not to exceed 100. Pages must be numbered. There shall be no other documents, albums, books, plans, etc. accompanying the submittal. 2. The actual submittal shall consist of one (1) original and two (2)duplicate copies of the entry. 3. All copies of application must be received before S;00 pm on April 15, 1993 to SWANA, 8754 Georgia Avenue, Suite 140, Silver Spring, MD 20910. No late applications will be accepted. 4. All original photographs are to be in color and must be taken between January 1, 1993 and April 15, 1993, (Photos demonstrating before/after features are an exception.) Photographs must have descriptive captions, including the data the photo was taken. Color copies of photos are allowed for duplicate entries. Photos are limited to two(2) per page for 3' x 5' photos,and one (1) per page for 5' x 7' photos. Photos and captions must be on the same page. 5. The facility must have received waste for at least 12 months prior to January 1, 1993. G. The completed LDAP 93 APPLICATION FORM must appear as the first item in the notebook. 7• Applications must contain all information as described in the next section, APPLICATION FORMAT. APPLICATION FORh1&T Each section of the application must be clearly separated with dividers and/or tabs. Where appropriate, submit discussions, along with schematics, photographs, etc. Slides and videotapes are not acceptable, however, award recipients may be requested to submit such material for future SWANA, presentations. Failure to submit any of the following required sections in the order listed shall invalidate the submittal. 1. LEAP 93 Application Form 2. Table of Contents giving page numbers for each section. 3. Section 1: Introduction -must include the following: a. Lauer dated no earlier than January 1, 1993 from the site's regulatory agency, confirming compliance with permit requirements, and establishes the full term of complianca longevity. The letter must reference gas and odor emissions relative to permit requirements, and if leachate has ever impacted the groundwater or surface water beyond the property boundary. Note: We recommend that you contact your state regulatory agencyconceznfliij"W aforementioned issues prior to completing the application. An application with a less than positive response Will not be eligible for an award. b. Letter from the consulting engineer or design engineer elated no earlier than January 1, 1993, confirming that construction of the site according to the design has been achieved over the past 24 months. The letter must reference slope contours, liner and leachate collection installation. C. Discussion of the history of the site and the aim of the site(in total acres, landfill ares, tonnage) 4.
