Page 3, Part 6.C- Date for Report of Waste Discharge needs to be change to 12/06/02 <br />(WDR# RS -2002-0219). <br />Public Works needs to submit to EHD updated SJVAPCD Permit (EHD has a copy of Permit <br />which expired on 11/30/06). <br />No Department of Health Services Permit needed. <br />11. Page 4, Part 8- Information for facility operator name and address where legal notice may be <br />served appears to be missing. <br />Please, submit to the EHD corrections that address the above noted items.
