All work related injuries and accidents, no matter how minor, must be reported
immediately to your supervisor.
It is imperative that all employees become thoroughly familiar with the above safety rules.
Failure to comply with safety rules or procedures, or failure to wear the appropriate safety
equipment, will result in disciplinary action up to and including termination.
DISCIPLINARY PROCED URES
Employees who fail to comply with safety rules will be subject to disciplinary action up to and
including termination. Supervisors will follow the normal disciplinary procedures as follows:
1. Verbal counseling—the first step. Must be documented in the employee's personnel
file.
2. Verbal or written warning—the second step, outlining nature of offense and necessary
corrective action.
3. Suspension with out pay—the third step or a separate disciplinary action resulting
from a serious violation.
4. Termination if an employee is to be terminated, specific and documented
communication between the supervisor and the employee, as outlined, must have
occurred. Termination may be the first step in the case of willful and flagrant
disregard for safety rules endangering the employee or others.
Supervisors will be subject to disciplinary action for the following reasons.
1. Repeated safety rule violations by their department employees.
2. Failure to provide adequate training prior to job assignment.
3.
