HSC 25508(a)(1) Failed to electronically submit the Business Activities or Owner/Operator <br /> Identification page. <br /> OBSERVATION:The Business Owner/Operator Identification form was not submitted or <br /> complete. According to Jason Torres,Operations Manager, Corey Dehl, who is listed on the <br /> Business Owner/Operator Identification form as the Primary Emergency Contact and the <br /> Environmental Contact, no longer works for the facility. <br />
