T Y <br />Time In: 10:15 am <br />Time Out: 10:30 am <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: IHOP Date: 04/29/2021 <br />Address: 1230W KETTLEMAN LN, LODI 95240 <br />Owner/Operator: LAXMI US 1234 FOOD CORP Telephone: (209) 369-1361 <br />Program Element: 1600- FOOD PROGRAM Complaint #: 000053732 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT ALLEGES THERE ARE LIVE ROACHES IN THIS FACILITY AND THE FILTERS FOR THE SODA <br />SYRUPS HAVE BEEN EXPIRED FOR FIVE YEARS <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION -- TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />I went to the facility and spoke to an employee who said they were not aware of the complaint. <br />Per employee the soda machine was addressed last week. <br />Per employee the facility was serviced by pest control on Sunday and pest control service to return and service <br />again tonight. <br />I inspected kitchen and did not observe "live roaches" at this time. <br />Signature not captured due to COVID <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
