SANdOAQUIN <br /> Environmental Health Department <br />CT Ot1 NTY <br />Time In: 1:22 pm <br />Time Out: 2:00 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: IHOP Date: 05/05/2021 <br />Address: 1230 W KETTLEMAN LN, LODI 95240 <br />Owner/Operator: Telephone: (209) 369-1361 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: C00053738 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />LIVE ROACHES IN RESTROOM, COMPLAINANT WOULD LIKE A CALL BACK FROM INSPECTOR ASAP. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION -- TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />I went to the facility and spoke to the Person In Charge who was aware of complaint. <br />Per Person In Charge the facility is currently undergoing treatment to abate the pest problem. <br />Facility was teated last Wednesday and last Thursday. <br />Treatment to occur again tonight. <br />I observed one dead adult German cockroach in women's restroom and one juvenile on the tile wall. <br />I observed the restroom wall in women's restroom. <br />The wall is also the ware wash room wall. <br />I observed ware wash room. Flooring is dry at this time. <br />Ensure that flooring is dry and clean to prevent the harbor age of pests. <br />I also observed the kitchen area and around the cooking equipment. <br />Areas have been cleaned for the exception of the far end (which is in the process per manager). <br />Continue with cleaning and sanitizing of these areas. <br />Ensure that areas are free of grease, food, and water to prevent the harbor age of pests. <br />Per employees there were also a few seen in the dining area. <br />I inspected the diningarea(s) near exit door near restroom and booth near north window. <br />I did not observe cockroaches in these areas at this time. <br />Continue with pest control and treatment. <br />Operator to provide me with copy of pest control receipts. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
