The following is an itemized list of hazardous materials violations that have not been <br /> addressed for JACK IN THE BOX as of December 06 , 2021 . <br /> Open violations from July 29 , 2021 inspection <br /> Violation #6 - Failed to electronically submit a site map with all required content. <br /> 1 . OBSERVATION The site map has not been annually certified/submitted as a part of the business plan . The site <br /> map was not submitted for 2021 . The site map has not been submitted since April 20 , 2020 . <br /> REGULATION GUIDANCE: Per H &SC 25508 . 2 , the business owner/operator shall annually review and certify that <br /> the business plan is complete , accurate , up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code . An annual electronic submittal satisfies these requirements . The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> CORRECTIVE ACTION : Immediately log into the CERS at http : //cers. calepa . ca . gov/, review and submit/certify the <br /> current information . <br /> . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . <br /> 2 . The adjacent street to the south , Charter Way is not shown . <br /> 4 . Access and exit points from the street. <br /> 5 . Emergency shutoffs . <br /> REGULATION GUIDANCE: A site map shall contain a north orientation , loading areas , internal roads , adjacent <br /> streets , storm and sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous <br /> material handling and storage areas , and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as " NA" . <br /> CORRECTIVE ACTION : Immediately log into the California Environmental Reporting System ( CERS ) at <br /> http : //cers . calepa , ca . gov/, upload the correct or updated information , and submit to the EHD for approval . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #9 - Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION The business plan information was not reviewed and resubmitted in the California Environmental <br /> Reporting System (CERS ) annually by the due date , January 15 . The annual submittal has not been completed for <br /> 2021 . The submittal for 2018 was completed on 2/20/ 19 , for 2019 the plan was submitted on 2/28/ 19 , and for 2020 <br /> the plan was submitted on 4/20/20 . <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year . <br /> CORRECTIVE ACTION : Immediately log into the CERS at http : //cers. calepa . ca . gov/ , enter the correct or updated <br /> information , and submit to the EHD for approval . <br /> Page 2 of 3 <br />
